The MAP program provides tribal citizens assistance with up to two episodes each year for unmet medical care not to exceed $2,500 per incident. The MAP program is a supplement to contract health services designed to provide necessary medical services to support on-going care from a CNDH provider to maximize the medical treatment plan.

Elder / Non-Elder Prescription Program
The MAP program also includes prescription programs for elders, non-elders and Chickasaws living outside of tribal jurisdictional boundaries.

Required applications for MAP are available at the address above, can be sent via email or may be obtained at all CNDH clinics.

Application for MAP must be completed within 180 days from the incident / injury / illness.

You Provide:

  • Chickasaw citizenship card
  • Copy of insurance
  • Most recent original statements, script and/or referral
  • If referred, must provide a copy of denial letter from contract health

Eligibility:
MAP

  • Must have a chart that has been active in CNDH within the past three years
  • Service must not be provided by CNDH facilities
  • Must have a medical necessity

Elder Prescription Program

  • 60 years of age or older
  • Chickasaw citizen with a Chickasaw Nation citizenship card
  • In need of a prescription medication that is not provided by the Chickasaw Nation pharmacy
  • Prescription is from your CNDH primary care physician
  • All refill orders must be ordered through the CNDH pharmacy - (580) 421-4569.

Non-Elder Prescription Program

  • Chickasaw citizen with a Chickasaw Nation citizenship card
  • (Under the age of 12- mother’s or father’s card(s) must accompany child’s CDIB
  • In need of a prescription medication that is not provided by Chickasaw Nation pharmacy
  • Prescription from your CNDH primary care physician
  • All refill order must be ordered through the CNDH pharmacy - (580) 421-4569.

Hours of Operation:
Monday thru Friday, 8 a.m. to 5 p.m.

To be eligible to participate in the MAP program, citizens must have an active chart within CNDH. (An active chart is defined as having received services within the past three years from the date of application.) Citizens must also have a referral denial from Medisaw. The MAP program is utilized as a last resort; therefore, alternate resources must be exhausted. The program does not cover services that are provided within the CNDH.

The Chickasaw Nation Division of Health offers a variety of programs and services at several convenient locations throughout our 13-county jurisdiction. The Chickasaw Nation Medical Center (CNMC) is located in Ada, Oklahoma and outlying clinics are located in Ardmore, Durant, Purcell and Tishomingo.
