About:

The purpose of the farmers' market nutrition program is to increase the awareness and utilization of local farmers' markets and to provide an additional nutritional benefit to eligible participants. 

The Senior Farmers' Market Nutrition Program (SFMNP) provides $50 of coupons for the purchase of fresh fruits and vegetables from authorized farmers' markets and farm stands throughout the Chickasaw Nation and surrounding areas.  Additional benefits include nutrition education and food demonstrations.  You may qualify for benefits from the Chickasaw Nation Senior Farmers' Market Nutrition Program if you:

• Meet income requirements
• Receive services from the Chickasaw Nation Division of Health
• Native American 55 years of age or older and a CDIB card holder
• Non-Native American 60 years of age or older living in a Native American household
• Fully disabled non-Chickasaw citizen under 60 years of age living in housing for elderly with congregate nutrition services
• Fully disabled Chickasaw Citizens (as determined by SSI)

Applications can be received by mail or picked up at WIC and FDP sites in Ada, Ardmore, Purcell, Tishomingo, Sulphur, Pauls Valley, and Duncan. 

The Chickasaw Nation Farmers' Market checks for seniors will be issued by mail once an application is dropped off at one of the above locations and processed. 

Program applications are accepted May 1- September 30 each year.

You Provide:

• CDIB or citizenship card
• Proof of 100% disability (for disabled Chickasaw participants only)
• Income estimate for household

Eligibility:

• WIC program participants
• Native American seniors (55 and older) who meet income guidelines
• Fully disabled Chickasaw individuals (as determined by SSI)
• Non-Native American seniors (60 and older) residing in a Native American household who meet the income guidelines
