The Family Practice Center (FPC) is designed to provide comprehensive health care to the entire family with one primary provider.

The Family Practice Center is staffed with six physicians and one physician assistant.  Patients are allowed to choose their provider.

The Ada FPC offers a full service laboratory, pharmacy and radiology department.

Hours of Operation:  Monday-Friday, 9 a.m. - 4 p.m., by appointment only

Eligibility
To be eligible for services you must have a CDIB from a federally recognized tribe.
