CASE MANAGEMENT

When a patient has been diagnosed with a medical condition that requires long-term care, the cost of medical supplies, equipment or other services can add up quickly, creating a hardship for many families.  At CAIHF, patients are screened to identify as many alternate resources as possible that will help with part of the cost.  Patients are informed of available programs, eligibility requirements and procedures on how to apply.
