Student Tutoring Allowance Assistance
About
The Chickasaw Nation Student Tutoring Reimbursement Program is funded by the Chickasaw Nation and provides at-risk Chickasaw students, in grades one through 12, assistance to improve their academic performance.
Eligible students must be on an individual education plan (IEP) or at risk with failing or declining grades in the subject for which assistance is being requested. A referral letter from his/her teacher or school official must be provided.
Reimbursement is for tutoring in basic skill subjects only (English-based reading and writing, math and science). Reimbursements through this program shall not exceed $600 per school year.
You Provide
- Chickasaw citizenship card
- Student Tutoring Reimbursement Program Application (Form 900)
- Tutoring Allowance Assistance Log (Form 900L)
- Copy of student’s grade report
- Supporting documentation
- Proof of IEP and/or referral letter from teacher or school official
- Tutoring invoice (indicating payment received
Eligibility
- Chickasaw citizen
- Grades 1-12
- Failing or declining grades (C and below) in the subject for which assistance is being requested
Other Information
Complete application must include the following: the tutoring assistance log, a copy of student’s grade report with a grade of C or below and supporting documentation (tutoring invoice showing payment received, statement of need from teacher and/or proof of IEP) before eligibility for the reimbursement program can be determined.
Reimbursement will not be provided for school tuition or online course fees, book rental, materials and supplies fees, or for tutoring assistance in advanced placement courses or concurrent college level classes.
