Other Information:
Submitting a verified report card:
When the division of education receives an application, it is entered in the database and the department advises the parent of their child's eligibility for the program. At the end of each six or nine week grade period, a duly verified copy of the child's report card must be sent to the division of education for processing.
The copy must establish that the child is eligible for an incentive and must be verified as a true and correct copy by the child's teacher, counselor or principal. If the report card has not been verified, the child will be ineligible for an incentive. Please type or write the following on the copy of the report card and obtain the proper verifying signature.
This is a true and correct copy of (student's name) report card for the period ending on the _______ day of ________, 20__.
