The Chickasaw Nation Health System ANNUAL FLU SHOT CLINIC

October 5th – 9th, 2009
Monday, October 5, 2009 – Wednesday, October 7, 2009 from 9 a.m. – 7 p.m.
Thursday, October 8, 2009 - Friday, October 9, 2009 from 9 a.m. – 5 p.m.

The flu shot clinic will be located in the main conference room at Carl Albert Indian Health Facility (CAIHF), 1001 N. Country Club in Ada. In addition, Outpatient Pharmacy at Carl Albert Indian Health Facility will conduct continued flu shot clinics for those who were unable to attend during the provided times listed above.

Monday, Oct. 19, 2009 from 10 a.m. – 3 p.m.
Monday, Oct. 26, 2009 from 10 a.m. – 3 p.m.

The flu vaccine will be offered to all Native Americans.

To participate, you must provide the following information at the main registration desk, located in the lobby of CAIHF for the annual flu shot clinic, or at Outpatient Pharmacy for the continued clinic dates. Adults must provide a CDIB or Tribal Identification Card with blood quantum listed. For a minor child, you must provide parent’s CDIB or Tribal Identification Card with blood quantum listed and a copy of a state certified birth certificate.
